Where are you located?
Our address is 708 Washington Street (Rt. 120), Woodstock, IL 60098. We are approximately three blocks west of the Woodstock Dairy Queen.

What are your hours of operation?
We are open Monday and Wednesday from 8 am-5 pm. Tuesday and Thursday we are open from 8 am-6 pm. We have limited Saturday hours available for evaluations.

Do you serve patients living outside of McHenry County?
Because of our funding, our focus is McHenry County. However, if you live outside of McHenry County and wish to receive services from us, please give us a call so we can speak further.

How long is an evaluation?
Please schedule one hour for an evaluation.

How long is each therapy visit?
Typically, each visit is one hour.

What types of therapy do you offer?
We offer physical, occupational and speech therapies.

How do I obtain your services?
Patients must have a prescription from their physician to receive services.

Do you accept public aid?
Yes we do.

What types of insurance do you accept?
We accept Medicare, Medicaid and commercial insurance. A verification of insurance benefits will be performed prior to scheduling your first appointment with us. If you do not have any type of insurance or medical assistance, we offer a sliding scale fee based on your income.

Do you have staff that will treat patients in their home?
Our physical, occupational and speech therapists will treat homebound patients. Please call for more information.

Do you speak Spanish?
Yes, a few members of our staff do speak Spanish. We also have access to a language line that provides interpreter services.

Do you accept equipment donations?
On occasion, we accept equipment donations (wheelchair, walker, etc.). However, our storage space is limited. If you have a piece of equipment you would like to donate, please give us a call. If we are unable to accept the donation, we will do our best to direct you to another social service agency that is able to take these types of donations.

Do you loan out equipment? Is there a charge?
We do have some equipment (wheelchairs, walkers, crutches, shower seats, etc.) that we are able to loan out. If you are looking for something on a temporary basis, please give us a call and we will do our best to accommodate you. There is no charge for this service.

I am considering a career in PT, OT or ST. Can I shadow one of your therapists?
We do allow those that are currently students in one of these fields to shadow our therapists. Please call for more information.

Do you provide transportation?
No, we do not provide transportation. There is a PACE Dial-a-Ride bus system available by calling 1-800-451-4599. Faith In Action is a non-profit organization that provides no cost rides to seniors. You can contact Faith In Action by calling (815) 455-3120.
